Job Opportunity – Registered Nurse, Outpatient Case Management – Napa, CA 94559

Start Date: August 10, 2026

Location: Napa, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 17:00), no weekends or call required. Average daily census of 72 participants. Patient ratio approximately 1:40.

Highlights:

• Work in a licensed Adult Day Health program serving frail elderly, younger disabled adults, and individuals with Alzheimer's Disease and related dementia
• Provide chronic disease case management for conditions including diabetes, hypertension, CHF, COPD, and degenerative neurological diseases
• Collaborate with an interdisciplinary team including physical therapy, social services, interpretation services, and transportation support
• Focus on reducing emergency department visits and hospitalizations while preventing premature nursing home placement
• Provide caregiver education and support in a specialty dementia care environment
• Free parking and mileage reimbursement at IRS standard rates provided

Requirements:

• Active California RN license required at time of start
• Minimum 1 year of prior RN experience required; clinic, primary care, pre/post-operative, or home health settings preferred
• Current BLS and First Aid certifications required
• Experience with care coordination and disease management preferred
• Reliable transportation required
• Two professional references covering 1 year within the last 3 years
