Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: August 10, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Weekend rotation required (every other weekend). Patient ratios: 1:25 on weekdays, 1:45 on weekends. 40 hours of orientation provided.

Highlights:

• Collaborative interdisciplinary team including Physical Therapy and Social Services
• Diverse patient population including neonates, adolescents, adults, and geriatrics

• Epic charting system utilized
• Up to 7 days of approved time off during assignment

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care Case Management or Discharge Planning experience required
• Hospital-based experience mandatory; behavioral health experience does not substitute
• Previous travel nursing experience required; first-time travelers will not be considered
• Epic charting system experience required
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, advance directives, POLST, and community resources
• Ability to organize and participate in family meetings and treatment plan development
• Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Two professional references covering 1 year within the last 3 years
