Job Opportunity – Registered Nurse, Case Manager – Mission Hills, CA 91345

Start Date: August 31, 2026

Location: Mission Hills, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), weekend rotation required to meet department needs. Patient ratio 1:20. 40 hours of orientation provided. Assignment duration is 91 days.

Highlights:

• Acute care facility setting with interdisciplinary support including social services and transportation
• Work with diverse patient populations including adolescents, adults, and geriatrics
• Utilize Epic charting system for computerized documentation
• Engage in care coordination, discharge planning, and family meetings
• No floating required within the unit
• Up to 7 days of requested time off available

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care hospital case management experience required
• Experience with care coordination and discharge planning in a hospital setting is mandatory
• Proficiency with Epic charting system required
• Knowledge of InterQual criteria, DRG, CMS guidelines, and plan of care development
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Experience with HMOs, IPAs, and working with complex patient populations
• Proficient in Microsoft applications
• First-time travelers accepted with clinical screening
• Travel nursing experience preferred
