Description

Job Opportunity – Registered Nurse, Case Management – Santa Rosa, CA 95405

Start Date: September 8, 2026

Location: Santa Rosa, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), 2 weekends every 6 weeks required. Caseload of approximately 20 patients. 40 hours of orientation provided. Floating between like units based on departmental needs.

Requirements:

Active Registered Nurse license (Compact license required at submission)
Minimum 2 years of case management experience required
Current work history within the past 2 years (HARDSTOP requirement)
BLS certification required
Epic charting system experience required
Experience with discharge planning, care coordination, and utilization of InterQual criteria
Competency in arranging DME, SNF placement, and ambulance transport independently
Experience with HMOs, IPAs, and insurance authorization processes
Ability to organize and participate in family meetings and treatment plan development
2 professional references covering 1 year within the last 3 years
Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
