Job Opportunity – Registered Nurse, Case Management – Apple Valley, CA 92307

Start Date: September 8, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), with every other weekend rotation required. Patient ratios: 1:25 on weekdays, 1:45 on weekends. 40 hours of orientation provided.

• Work with diverse patient populations including neonates, adolescents, adults, and geriatrics

• Epic charting system utilized for computerized documentation

• Opportunity to work with a collaborative care coordination team

Requirements:

• Active California RN license (Compact license accepted; must be licensed by start date)
• Minimum 2 years of acute care Case Management/Discharge Planning experience required
• Hospital experience mandatory; behavioral health experience does not substitute
• Previous travel nursing experience required; first-time travelers cannot be considered
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Proficiency with HMOs, IPAs, and knowledge of community resources including foster care, substance abuse treatment, mental health services, and advance directives
• Competent with independently arranging DME, SNF placement, and ambulance transport
• Experience with family meetings and treatment plan development
• Two professional references covering one year within the last three years
