Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: September 14, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), with every other weekend rotation to meet department needs. Patient ratios: 1:25 on weekdays, 1:45 on weekends. 40 hours of orientation provided.

• Work with diverse patient populations including neonates, adolescents, adults, and geriatrics
• Interdisciplinary team support including physical therapy and social services
• Epic charting system utilized

• No floating required within the unit

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care case management or discharge planning experience required
• Hospital experience mandatory; behavioral health experience does not substitute
• Seasoned travel nursing experience required (first-time travelers cannot be considered)
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Proficiency with HMOs, IPAs, and knowledge of community resources including foster care, substance abuse treatment, homelessness services, and mental health crisis intervention
• Competent in independently arranging DME, SNF placement, and ambulance transport
• Ability to organize and participate in family meetings and treatment plan development
• Experience with computerized documentation systems
• Two professional references covering one year within the last three years
