Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: September 21, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), with every other weekend rotation. Patient ratios: 1:25 on weekdays, 1:45 on weekends. 40 hours of orientation provided.

Requirements:

• Active California RN license (compact license accepted if converted by start date)
• Minimum 2 years of acute care Case Management or Discharge Planning experience required
• Hospital experience mandatory; behavioral health experience does not substitute
• Previous travel nursing experience required (first-time travelers cannot be considered)
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis intervention, advance directives, and POLST
• Ability to organize and participate in family meetings and treatment plan development
• Two professional references covering one year within the last three years
• Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
