Job Opportunity – Registered Nurse, Case Manager – Burbank, CA 91505

Start Date: September 28, 2026

Location: Burbank, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Patient caseload up to 25. Weekend requirement of one full weekend per four weeks, subject to department needs. No floating required.

• Work with adult and geriatric patient populations in a collaborative care coordination environment

• 40 hours of paid orientation to ensure successful onboarding
• Experience with Epic charting system required

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute medical Care Manager experience performing care coordination and discharge planning
• Epic charting system experience required
• Experience with HMOs, IPAs, and assessing patients for care coordination
• Knowledge of admission criteria, medical necessity determination, discharge planning, and disease management
• Familiarity with CMS guidelines, HIPAA regulations, CPT coding, and DRG classifications
• Two professional references covering one year within the last three years
• First-time travelers accepted with appropriate screening
