Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: October 5, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour (08:00 - 16:30). Patient ratios: 1:25 weekdays, 1:45 weekends. Every other weekend rotation required. No floating required.

Highlights:

• 6-month travel assignment at an acute care facility through April 5, 2027
• 40 hours of paid orientation to support your transition
• Work with interdisciplinary teams including Physical Therapy and Social Services
• Serve diverse patient populations including neonates, adolescents, adults, and geriatrics
• Business attire or scrubs accepted
• Epic charting system utilized

Requirements:

• Active California RN license (pending license accepted but must be active by start date)
• Minimum 2 years of acute care Case Management/Discharge Planning experience required
• Hospital-based experience mandatory; behavioral health experience does not substitute
• Previous travel nursing experience required (seasoned travelers only)
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Proficiency with HMOs, IPAs, and arranging DME, SNF, and ambulance transport independently
• Knowledge of adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis management, advance directives, and protective services processes
• Competency with computerized documentation systems
• Two professional references covering one year within the last three years
• Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Clean background check required
