Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: October 19, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Every other weekend required to meet department needs. Patient ratios: 1:25 on weekdays, 1:45 on weekends. 40 hours of orientation provided.

Highlights:

• 13-week travel assignment at an acute care facility serving diverse patient populations including neonates, adolescents, adults, and geriatrics
• Work with interdisciplinary team including Physical Therapy and Social Services
• Epic charting system in a professional environment with business attire or scrubs
• Enhanced winter rate period from October 2026 through February 2027
• Opportunity to utilize comprehensive case management skills including discharge planning, utilization review, and care coordination
• No floating required within the unit

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care case management or discharge planning experience required
• Hospital experience mandatory; behavioral health experience does not substitute
• Previous travel nursing experience required – first-time travelers cannot be considered
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, and various social services including foster care, substance abuse treatment, homelessness resources, and mental health crisis intervention
• Ability to organize and participate in family meetings and treatment plan development
• Proficiency with computerized documentation systems, specifically Epic
• Two professional references covering one year within the last three years
• Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Clean background check required
