Job Opportunity – Care Manager – Chico, CA 95926

Start Date: September 21, 2026

Location: Chico, California

Schedule: 8-hour shifts, 8:00 AM - 4:30 PM, weekends and weekdays. 40 guaranteed hours per week. 13-week assignment (91 days).

Highlights:

• Coordinate care and services for assigned patients throughout their episode of illness or treatment
• Collaborate with patients, families, physicians, care teams, and payors to achieve optimal clinical outcomes
• Utilize advanced clinical skills and nursing process principles for assessment, planning, intervention, and evaluation
• Evaluate quality and utilization cases, including admission/discharge criteria, readmissions, and Medicare compliance
• May include Emergency Department assignment with case management responsibilities
• Serve as patient advocate and resource throughout the entire care continuum

Requirements:

• Current California RN license
• Current CPR certification
• Minimum one year of recent Case Management experience in an acute care setting
• Must be W-2 employee of staffing agency
