Required Certifications & Licenses

• ARRT (American Registry of Radiologic Technologists)
• BLS (Basic Life Support)
• State License (CPDH Certificate required for RTs in California)

Compliance & Documentation Requirements

• Background Check within 45 days of start, covering all locations lived/worked in past 7 years
• Drug Testing: 10 Panel Drug Screen within 90 days of start (no instant tests)
• Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
• Immunizations & Health Documents:

  • TDAP (within 10 years, declination accepted)
  • Negative TB or Chest X-Ray (within 12 months, annual updates required)
  • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer, declination accepted
  • Hepatitis B vaccination or signed declination
  • Varicella - 2 vaccines or positive titer, declination accepted
  • Flu Vaccine during flu season (declination accepted)
  • COVID Vaccine or Declination
• Respirator Safety Clearance (RespSafety) required for travel candidates, updated annually
• Fit Mask Testing required for travel candidates (annual update)

Competency & Training Requirements

• Medication Test/Pharmacology Competency Exam (as applicable)

• Color Blind Testing (if requested)
• Epic Training Modules (completion required prior to or shortly after start)
• Providence WorkRamp Modules:

  • Core Modules for profession (RN/LVN/Allied)
  • Specialty-specific modules for applicable roles

• Float Policy Acknowledgement (may be required to float to other Providence facilities)

References & Employment History

• Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting (6 months for CNAs)
• Employment Application covering at least last 5 years (7 years for APN/Subcontractors)
• Skills Checklists updated annually (if applicable)

• Pending licenses are not accepted; license must be active at time of submission
• Background checks must be current and clear of open/active felony cases or multiple DUI charges
