**Join Our Dynamic Team as a Physical Therapist - Home Health!** **Job Description:** - Embrace the opportunity to make a real difference in the lives of patients in Solano County, California! - Prioritize start of care visits to ensure timely and effective patient support. - Supervise and mentor Physical Therapy Assistants, fostering a collaborative environment. - Submit mandatory next day reporting for start of care visits to maintain strong communication. - Ensure clinical notes and documentation are completed within 48 hours of each visit. - Maintain timely and open communication with your supervisor, intake, and office team. - Be responsive and available during normal business hours for questions and updates. - Enjoy a structured schedule of 40 hours per week, from Saturday to Wednesday. - Thrive in a supportive environment with a comprehensive training program: 3 full days, including one in-house and two in the field. - Utilize the Kinnser documentation system for streamlined processes. - Benefit from issued equipment, including an iPad and Verizon One Talk. - Receive mileage reimbursement at a generous rate of $0.655 per mile (from first to last patient). - Reimbursement for parking and tolls paid for your convenience. - Option for a company car with a decreased bill rate available. **Productivity Requirement:** - Meet a productivity requirement of 30 points per week, with each point equating to 1.33 hours of work. - Work with a diverse caseload and visit mix, averaging 8 hours per day and 40 hours per week. **Visit Type Point Values:** - Start of Care: 2 points - Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points - Revisit, NON OASIS DC: 1 point --- **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
