**Exciting Opportunity for MSW or RN Case Manager!** **Job Description:** - Join a dynamic healthcare team dedicated to making a meaningful impact in the lives of patients! - Work in a culturally rich environment serving diverse communities, including Navajo, Hopi, and Southern Paiute tribes. - Utilize your skills as an Outpatient/Inpatient Social Worker or Registered Nurse Case Manager. - Design and manage a continuum of care focused on empowering clients towards self-sufficiency and demonstrable outcomes. **Key Responsibilities:** - Provide direct supervision and support to identified high-risk patients. - Conduct assessments, service planning, and resource acquisition. - Monitor patient progress and respond proactively to emerging needs. - Collaborate with community social service providers and healthcare professionals to achieve patient goals. - Conduct utilization review for inpatient and outpatient services. - Ensure medical necessity for referrals to specialty providers and assist with effective resource management. **Necessary Qualifications:** - **Education:** - MSW Social Worker: Master’s Degree in Social Work (MSW), OR - RN Case Manager: Associate’s Degree in Nursing (must obtain a Bachelor’s within 2 years of hire). - **License/Certification:** - Maintain current Basic Life Support (BLS) certification. - MSW: Licensed Master Social Worker (LMSW) or LCSW in any U.S. state, Puerto Rico, or U.S. territory. - RN: Valid and unrestricted Professional Nursing License in any U.S. state, Puerto Rico, or U.S. territory. - **Experience:** - A minimum of three (3) years of clinical care experience. - Knowledge of discharge planning, case management, or utilization review processes. - Familiarity with electronic health record systems. **Shift Information:** - 40 hours per week (options for 5-8 hour shifts or 4-10 hour shifts). - Weekend rotation and call coverage required. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
