Here’s what you can expect in this exciting role: - **Collaborative Environment:** Work closely with patients, families, physicians, and a multidisciplinary team to develop tailored care plans from admission through discharge. - **Optimize Patient Flow:** Take charge of ensuring smooth transitions and efficient patient progression through the care process. - **Assess and Advocate:** Perform comprehensive assessments to address social, emotional, and financial factors affecting patients, while advocating for their treatment choices. - **Discharge Planning:** Initiate and manage discharge planning from the moment of admission and provide support with post-acute care options. - **Resource Coordination:** Facilitate connections to necessary resources, including acute rehabilitation, skilled nursing placements, home health, and community resources. - **Patient-Centric Approach:** Provide education and direction that aligns with patients' goals of care, and promote understanding of their treatment plans. - **Performance Improvement:** Participate in initiatives focusing on length of stay, resource utilization, and readmissions to enhance care delivery. - **Professional Development:** Engage in continuous education and skills competency, and be a part of a supportive team that encourages growth. **Job Requirements:** - **Education:** BSN or MSW required; experience can substitute for formal education. - **Experience:** Minimum two years of clinical experience; one year in an acute care setting preferred. - **Licenses/Certifications:** RN or LMSW in New Mexico required; LBSW accepted with significant experience.
