Case Manager (Registered Nurse) - Travel/Contract

Location: Mission Hills, California
Contract Length: 13 weeks
Shift: Day (5x8 hours), 8:00 AM - 4:30 PM
Hours per Week: 40

Job Summary

Seeking a Registered Nurse Case Manager with acute care hospital experience to perform care coordination and discharge planning. This is a travel/contract position at an acute care facility with no floating expectations. Weekend rotation is required to meet department needs.

Required Qualifications

Valid RN license in California
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Minimum 2 years of acute care case management experience
Acute care case management experience within the last year
