Physical Therapist - Home Health

Location: Sunnyvale, California (Territory includes Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale)

Employment Type: Contract
Contract Length: 26 weeks (approximately 6 months)
Shift: Days, 8:00 AM – 5:30 PM (flexible based on patient availability)
Weekend Requirement: One weekend day per month
Hours per Week: 40

Job Summary

• Restore function to prevent or minimize disability following disease or injury.
• Help clients reach maximum performance/function within their capabilities.
• Evaluate and treat patients using medically prescribed physical therapy programs.
• May supervise Physical Therapy Assistants.
• Conduct initial admission/evaluation visits.
• Notify Clinical Team Manager of OASIS scores within established timelines.
• Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.
• Perform other duties consistent with skills and training.

Required Qualifications

• Valid Rehabilitation Therapist license in California
• Minimum 1 year of Rehabilitation Therapist experience within the last year
• Experience in a Home Health setting
• Physical Therapist experience
