Registered Nurse Case Manager - Contract
Location: Glenwood Springs, Colorado
Employment Type: Contract
Contract Length: 4 weeks
Job Description
- Full-time day position with weekend rotation, no on-call duties
- Schedule: 3 shifts per week, each 12 hours (total 36 hours/week)
- Assist inpatients and outpatients and their families with management of their plan of care across the continuum
Required Qualifications
- Valid Registered Nurse (RN) license in Colorado
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 3 years of Care Manager experience, including experience within the last year
- Experience with Epic electronic health record system
Additional Information
- Potential housing available: newly remodeled furnished studio and one-bedroom apartments near the hospital, including utilities and Wi-Fi (based on availability)
- No call responsibilities
- Ensure accurate Social Security number and date of birth for onboarding to avoid delays
