Physical Therapist
Location: Tuba City, Arizona
Employment Type: Contract
Contract Length: 13 weeks
Job Description
- Provide quality physical therapy services primarily to a Native American population.
- Manage and implement the Physical Therapy program.
- Perform physical therapy evaluation and treatment for inpatients and outpatients, from birth through geriatric patients.
- Provide specialty services including wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, assistive technology/adaptive equipment evaluation/treatment, and Durable Medical Equipment Prosthetic Orthotic Supply Program.
- Lead the physical therapy section including quality improvement, risk management, safety, policy and procedure development.
- Participate in Joint Commission preparation, staff education, reporting, staff meetings, and in-service training.
- Fulfill responsibilities listed on the Medical Staff Credentialing Core Privileges form.
- Work schedule includes Monday to Friday, 8am-5pm, with weekend rotation.
Required Qualifications
- Valid Rehabilitation Therapist license in Arizona.
- Minimum of 3 years of Rehabilitation Therapist experience, including experience within the last year.
- Experience as a Physical Therapist.
- Current Basic Life Support (BLS) certification issued by the American Heart Association.
- Must meet credentialing requirements to serve on the Medical Staff.
- Fingerprinting must be submitted and cleared prior to start.
Additional Information
- Orientation occurs every 2 weeks.
- COVID and flu vaccination exemptions may be accepted after submission of a declination form.
- Housing application must be submitted 2 weeks prior to start.
