Physical Therapist - Contract Position

Location: Petaluma, California

Job Overview

As a Physical Therapist, you will assess and treat patients to help them regain physical function and improve mobility. You will design and implement individualized treatment plans that promote independence and enhance quality of life. This role involves working collaboratively within an interdisciplinary healthcare team.

Key Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
• Provide physical therapy interventions to improve strength, mobility, and functional abilities.
• Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications.
• Collaborate with occupational therapists, nurses, and physicians to coordinate care and optimize patient outcomes.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and timely documentation of assessments, progress notes, and treatment plans.
• Ensure care plans comply with regulatory requirements and best practices in physical therapy.

Required Qualifications

• Valid Rehabilitation Therapist license in California
• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
• Minimum of 1 year of Rehabilitation Therapist experience, including experience within the last year
• Basic Life Support (BLS) certification
• Experience as a Physical Therapist in a healthcare setting
