Job Opportunity: Outpatient Registered Nurse Care Manager

Location: Napa, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Work in a licensed Adult Day Health program serving frail elderly, younger disabled, and people with Alzheimer's Disease and related dementia.
  • Program licensed for 100 patients/day; average daily census ~72.
  • Role involves chronic disease case management, monitoring medical conditions alongside interdisciplinary team (IDT) and primary care providers (PCP) in the community.
  • Provide caregiver education and support.
  • Goal: reduce emergency department visits and hospitalizations, prevent premature nursing home placement.
  • Use Turbo TAR EMR (specific to California Adult Day Health Care industry).
  • Patient ratio approximately 1 RN to 40 patients daily.
  • Facility type: Outpatient Clinic.
  • Attire: Any basic color scrubs.
  • Parking: Free.
  • Mileage reimbursement per IRS standard.

Schedule

  • Shift: Day shift, 5 days per week, 8 hours per day (8:00 AM – 5:00 PM).
  • No weekend requirement.
  • No on-call duties.
  • No floating outside scope of practice.
  • No block scheduling accommodations.

Required Qualifications

  • Valid Registered Nurse (RN) license in California.
  • Minimum of 1 year of case management experience, with case management experience within the last year required.
  • Clinic experience preferred.
  • Basic Life Support (BLS) certification issued by the American Heart Association or Canadian Heart and Stroke Foundation.
  • Pediatric Advanced Life Support (PALS) certification issued by the American Heart Association or Canadian Heart and Stroke Foundation.
  • NIH Stroke Scale (NIHSS) certification.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Patient Care & Documentation

  • Charting in Turbo TAR EMR system.
  • Daily patient ratio: 1:40.

Compliance & Background Requirements

  • Background check within 90 days of start, covering all locations lived/worked in past 7 years; must be updated annually.
  • Education verification required for nursing degree; international education requires additional documentation.
  • Drug screening: 9-panel drug screen within 12 months of start.
  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation.
  • Immunizations and health requirements:
    • Tetanus/Diphtheria/Tdap (within 10 years; declination accepted).
    • Negative TB test or chest X-ray within 12 months; annual updates required.
    • Measles, Mumps, Rubella (2 MMR vaccines or positive titer; declination accepted).
    • Varicella (2 vaccines or positive titer; declination accepted).
    • Hepatitis B vaccination series or signed declination.
    • Seasonal flu vaccine during flu season or declination.
    • COVID vaccination record preferred; declinations not required but unvaccinated must follow infection prevention protocols.
  • Respirator medical clearance required annually for travel candidates.
  • Fit mask testing required for travel candidates, updated annually.

Certifications & Competencies

  • BLS certification (AHA or Canadian Heart & Stroke Foundation).
  • PALS certification (AHA or Canadian Heart & Stroke Foundation).
  • NIH Stroke Scale (NIHSS) certification.
  • Additional certifications (ACLS, NRP, Chemotherapy, EFM, IV Certification, CPI, Management of Assaultive Behavior) as applicable per clinical track and facility requirements.
  • Medication/pharmacology competency exam as applicable.
  • Competency exams required as applicable by specialty.

Documentation & Onboarding

  • Employment application covering at least last 5 years of employment required.
  • Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting required.
  • Passport-style badge photo with neutral background required.
  • Completion of annual regulatory and specialty-specific training modules via LMS (Workramp).
  • EPIC training modules may be required depending on facility assignment.

Additional Notes

  • Facility may call off traveler up to 1 shift per pay period (every 2 weeks).
  • Confirm return-to-office (RTO) restrictions with hiring manager.
  • No floating to other facilities expected but float policy acknowledgment required.
