Job Opportunity: Registered Nurse – Care Coordination (Case Management)
Location: Sacramento, California
Contract Length: 8 weeks
Job Summary
Seeking an experienced RN for a travel contract in Care Coordination (Case Management) at an acute care hospital. The role involves discharge planning, utilization review, and managing complex socially-impacted discharges in an underserved area.
Required Qualifications
- Active California RN license
- BLS certification
- Minimum 3 years hospital experience, including acute level care coordination
- Proficiency with Midas, InterCall, Cerner, Curaspan, and Microsoft Office
- Experience with discharge planning and utilization review
- Prior acute care experience within California Dignity/CommonSpirit facilities is mandatory
- Must have 2 supervisory references: one within the last 12 months and one within the last 3 years
- Candidates cannot have been employed (full-time, part-time, or PRN) by the associated health system within the past year, except PRN through outside firms
Job Details
- Patient assignment ratio: 1:20
- Shift: 8-hour day shifts, 40 hours per week
- Option for 12-hour night shifts available only for candidates with ED background and willingness to work nights
- No overtime available
- Floating required to cover all adult hospital areas except women’s health (unless experienced)
- Orientation: 4 days webinar on charting + 2 days preceptor
- Dress code: Business casual conservative, no open-toed shoes, lab coats required, minimal to no perfume
- Must be adaptable, fast learner, flexible, with excellent attendance and ability to take breaks and lunches
Work Environment & Additional Notes
- Located in an underserved area with complex discharge needs including room and board placements, conservatorship, and under/uninsured patients
- Familiarity with placement agencies, Medicare/Medi-Cal guidelines for SNF and lower-level placements, and strategies for discharging patients with limited resources is preferred
- Weekend shifts every other week (EOW)
- 24/7 Respiratory Therapy and Pharmacy support available
Summary
This position is ideal for an RN with strong acute care coordination experience, especially in discharge planning for socially complex patients, who is comfortable working in a fast-paced, underserved hospital setting. Prior experience with the specified electronic systems and California acute care facilities is required.
