Case Manager

Overview

Location: Apple Valley, California
Employment type: Travel/Contract
Start date: ASAP
Contract length: 13 weeks
Shift: Day, 5x8 hours (8:00–16:30)
Hours per week: 40
Facility type: Acute care facility
Weekend requirement: Every other weekend
Floating: No
RTO limit: No more than 7 days

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
Case Management (2 years of experience)
Case Management experience within the last year
Acute Hospital Case Management Experience
Discharge Planning
One completed travel assignment
BLS issued by American Heart Association
NIHSS

Work Environment

Ratios: 1:25 on days; 1:45 on weekends
Dress code: Business attire or scrubs

Notes for Candidates

Behavioral health experience does not count toward the experience requirement
Pending license is accepted only if fully licensed by the start date
