RN Case Manager

Schedule
- Weekend coverage: 5 hours on one Saturday each month, with 5 hours taken off on one day during the week
- 36 hours per week
Required Qualifications
- Bachelor of Science in Nursing
- 5 years of Case Management experience
- Case Management experience within the last year
- Experience in a Hospital setting
- Discharge Planning
- Valid RN license in Colorado
- Current BLS
Preferred Qualifications
- Master’s degree in nursing
- Certification in case management, or willingness to obtain it within 2 years of employment
Key Responsibilities
- Develop, implement, and evaluate case management processes
- Assess, plan, coordinate, monitor, and evaluate patient health and human services needs
- Provide advocacy, communication, collaboration, resource management, and education to interdisciplinary team members
- Support appropriate, effective, and cost-efficient care for patients and populations
- Document case management plans and patient contacts clearly and concisely
- Collaborate with physicians, patients, families, staff, payers, and review organizations
- Educate physicians and staff on level-of-care and utilization issues
- Maintain knowledge of case management, discharge planning, and nursing issues through continuing education
