RN Case Management
Job Overview
- Travel RN role in San Leandro, California
- Monday–Friday, 8:30 AM–5:00 PM
- 40 hours per week
- Contract length: 13 weeks
Required Qualifications
- Valid California RN license
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS issued by the American Heart Association
- 2 years of Acute Care Case Management experience
- Acute Care Case Management experience within the last year
- Experience in a Long Term Acute Care setting
- Short Stay experience

- Care coordination and utilization management
- Assessment, care planning, implementation, and evaluation
- Discharge planning and continuity of care
- Patient advocacy and resource utilization
