Physical Therapist - Home Health
Location
- Oakland, California
- Territory: Sacramento, CA, with expansion as needed
Schedule
- 40 hours per week
- Monday-Friday
- One weekend a month rotation
- Day shift
Contract Length
- 13 weeks
Required Qualifications
- Valid Physical Therapist license in California
- 1 year of Physical Therapist experience
- Physical Therapist experience within the last year
- BLS
- Experience in a Home Health setting
Responsibilities
- Prioritize start of care visits
- Supervise Physical Therapy Assistants
- Submit start of care visit reports the next day
- Complete clinical notes and other documentation within 48 hours of the visit
- Maintain timely communication with supervisor, intake, and office
- Be responsive during normal business hours
Training
- 3 full days of training: one in-house and two in the field

Reimbursement / Logistics
- Mileage reimbursement: $0.65.5 per mile from first to last patient
- Parking and toll reimbursement paid
- Company car option with a decreased bill rate
