Description
The position is day shift, 8:00 AM–4:30 PM, Tuesday–Saturday, totaling 40 hours per week. No on-call requirement.
Candidates must have a minimum of 1 year of acute care case management experience, with strong inpatient case management and complex discharge planning experience. Cerner experience is required, along with an active RN license in the state of practice. Case Management certification is preferred.
Experience working strike-force assignments for Kaiser Permanente or HCA California is highly preferred, particularly for candidates who are accustomed to quickly adapting to new environments, require minimal training, and can independently manage complex discharge planning.
