📅 13-Week Contract
🕒 Shift: Monday – Friday, Days | Rotating Weekend Coverage
⏰ Guaranteed Hours: 40 Hours/Week

✨ Job Summary:
Seeking a dedicated Physical Therapist to evaluate, plan, direct, and administer therapy services in inpatient, outpatient, and home health settings. The PT will focus on restoring function, improving mobility, relieving pain, and increasing patient independence through evidence-based treatment plans.

🔹 Must-Have Skills:
✔ Active Illinois Physical Therapist License
✔ Valid Driver’s License
✔ Proof of Personal Auto Insurance
✔ BLS Certification
✔ Strong communication and customer service skills
✔ Ability to work independently and prioritize tasks

🔹 Experience Required:
✔ Minimum Less than 1 year of PT experience required
✔ Minimum 1 year preferred
✔ Home Health / Outpatient / Inpatient experience preferred

🔹 Key Responsibilities:
• Perform patient evaluations and develop individualized treatment plans
• Administer therapy modalities and skilled therapy techniques
• Improve mobility, strength, and functional independence
• Instruct patients on adaptive and assistive equipment use
• Supervise Physical Therapist Assistants (PTAs)
• Collaborate with rehab team and providers for optimal outcomes
• Maintain timely and accurate documentation in EMR
• Participate in quality improvement and patient safety initiatives
