Position Overview
Seeking an experienced Registered Nurse (RN) to provide field-based nursing care to elderly and medically complex participants. The RN will conduct in-person assessments, provide wound care and other skilled nursing services, respond to urgent and emergent needs, and coordinate care with providers, families, facilities, and healthcare partners.
Responsibilities
Conduct in-person participant assessments and determine appropriate levels of care
Perform routine, acute, and non-acute home visits
Provide wound care, IV hydration, lab draws, nursing assessments, and other ordered treatments
Respond promptly to calls from PCPs, triage teams, and support staff
Document all calls, visits, assessments, and follow-up activities in the EMR
Provide participant and family education and support
Coordinate care with physicians, pharmacies, hospitals, SNFs, ALFs, DME providers, paramedics, and other partners
Initiate emergency treatment when necessary
Implement and document new orders when indicated
Document falls, medication errors, incidents, grievances, and other quality-related events
Collaborate with all members of the participant's care team
Participate in staff meetings, orientation, in-service education, and quality improvement activities
Adapt to changing workloads and team priorities
Communicate concerns and issues to leadership
Requirements
โ Active Michigan RN license
โ RN diploma or degree from an accredited nursing program
โ Associate degree required; BSN preferred
โ Minimum 3 year of experience working with frail, elderly, or long-term care populations
โ 2 year of home care experience preferred
โ 2 year of wound care experience preferred
โ Current AHA CPR certification or willingness to obtain
โ Reliable transportation
โ Valid Michigan driver's license and auto insurance
โ Must be 18+ years old
โ Willingness to meet required vaccination requirements
โ Strong communication, critical-thinking, and assessment skills
