Responsibilities

✅ Manage and coordinate hospice patient care plans

✅ Collaborate with physicians, patients, families, and caregivers

✅ Conduct ongoing clinical assessments and monitor patient progress

✅ Provide education on care techniques and symptom management

✅ Document care and communicate effectively with the interdisciplinary team

Requirements

✔ Active RN License in Vermont & New Hampshire OR Compact RN License

✔ Graduate of an accredited nursing program

✔ Minimum 2 years of RN experience

✔ BLS/BCLS Certification

✔ Hospice, Oncology, Medical-Surgical, or Home Health experience preferred

CloudinfraIT Inc Job ID #297084. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
