Travel Home Health RN (Case Manager) – Full Time | Barre, VT

Position Overview:
Seeking a compassionate and independent Home Health Registered Nurse (RN) to provide skilled nursing care in patients' homes throughout Central Vermont. This role includes case management responsibilities and focuses on delivering individualized, goal-oriented care while promoting patient independence and quality of life.

As a community-based, nonprofit healthcare organization, Central Vermont Home Health & Hospice is dedicated to providing exceptional care regardless of a patient's financial circumstances while supporting the health and well-being of the communities it serves.

Responsibilities:
Provide skilled nursing care in the home setting according to physician orders.
Conduct comprehensive patient assessments and develop individualized care plans.
Utilize the nursing process to plan, implement, and evaluate patient care.
Serve as Case Manager when assigned as the lead discipline.
Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
Educate patients and caregivers on disease management, medications, and treatment plans.
Complete timely and accurate clinical documentation using Homecare Homebase (HCHB).
Monitor patient progress and adjust care plans as appropriate.
Ensure compliance with agency policies and regulatory standards.
Requirements:
✅ Active Vermont RN License
✅ Minimum 3 year of nursing experience required in Home Health
✅ BLS Certification required
✅ Strong assessment, communication, and case management skills
✅ Experience with Homecare Homebase (HCHB) preferred

Contract Details
16-week travel assignment
Full-time schedule: Tuesday through Saturday
Guaranteed 36 hours per week
EMR: Homecare Homebase (HCHB)
No holiday coverage required
Compensation & On-Call
Overtime paid after 40 hours/week at 1.5x rate
2-hour minimum call-back guarantee
