🚨 Travel Hospice RN Case Manager Opportunity 🚨

We are seeking an experienced Hospice Registered Nurse Case Manager for a 13-week travel assignment.

📍 Montrose, Colorado
🗓 Start Date: July 13, 2026
⏳ Contract Length: 13 Weeks
🕒 Shift: Monday–Friday | 8:00 AM – 5:00 PM
📅 Rotating On-Call & Weekend Coverage Required
🏥 Specialty: Hospice Case Management

**Requirements:**
✔ Active Colorado RN License
✔ Minimum 3 Years of RN Experience
✔ **Hospice Experience Required**
✔ Strong Assessment & Symptom Management Skills
✔ Excellent Communication and Care Coordination Skills
✔ Ability to Work Independently in Home & Community Settings

**Responsibilities:**
• Manage an assigned caseload of hospice patients
• Perform comprehensive patient assessments and develop individualized care plans
• Coordinate care with physicians, social workers, chaplains, and interdisciplinary team members
• Provide education and emotional support to patients and caregivers
• Manage pain and symptoms to maximize patient comfort and quality of life
• Document patient care accurately and maintain timely clinical records
• Provide care in patients' homes, assisted living facilities, nursing homes, and other community settings
• Participate in rotating on-call and weekend coverage as assigned
