🚨 Travel RN – Care Coordinator Opportunity 🚨

We are seeking an experienced Registered Nurse Care Coordinator for a 13-week travel assignment.

📍 Chico, California
🗓 Start Date: ASAP
⏳ Contract Length: 13 Weeks
🕒 Shift: 8:00 AM – 4:30 PM (8-Hour Shifts)
📅 Weekdays & Weekends
🏥 Specialty: RN Care Coordinator / Case Management

**Requirements:**
✔ Active California RN License
✔ Current BLS/CPR Certification
✔ Minimum 3 Year of Recent Acute Care Case Management Experience
✔ Strong Care Coordination & Discharge Planning Experience
✔ EPIC EMR Experience Preferred
✔ Excellent Communication and Critical Thinking Skills

**Responsibilities:**
• Coordinate patient care throughout the entire episode of illness or treatment
• Collaborate with physicians, patients, families, multidisciplinary teams, and insurance providers
• Perform comprehensive patient assessments, care planning, interventions, and evaluations
• Facilitate safe, timely, and cost-effective discharge planning
• Review admissions, observation stays, readmissions, and utilization management criteria
• Monitor patient progress and advocate for optimal clinical outcomes
• Coordinate community resources and post-acute care services
• Maintain accurate documentation and communication using EPIC EMR
• Support quality improvement initiatives and resource utilization management
• Serve as a patient advocate throughout the continuum of care
