🚨 Home Health RN Travel Assignment – Rutland, VT 🚨
📍 Location: Rutland, Vermont
🗓️ Contract Length: 13 Weeks
🕒 Schedule: Monday–Friday, 8:00 AM–4:00 PM
⏱️ Guaranteed Hours: 37 hours/week
📞 On-Call & Weekends: Some required
💻 EMR: McKesson
We are seeking an experienced Home Health Registered Nurse for a 13-week assignment. This role requires strong clinical assessment, case management, OASIS documentation, and patient education skills.
Key Responsibilities:
• Complete comprehensive patient and caregiver assessments and develop individualized plans of care.
• Manage assigned caseloads and coordinate care with physicians, providers, therapists, social workers, vendors, and interdisciplinary team members.
• Complete accurate and timely OASIS documentation, visit notes, care plans, and required agency, state, and federal forms.
• Provide skilled home health nursing care, including IV therapy, wound VAC management, complex wound care, infection control, and medication management.
• Educate patients and caregivers on medications, treatment plans, discharge instructions, and available services.
• Supervise and collaborate with LPNs and home care aides as needed.
• Support safe patient discharges, transfers, incident reporting, peer reviews, case conferences, and interdisciplinary team meetings.
Requirements:
• Minimum 3 years of Home Health RN experience
• OASIS experience required
• Active RN license or eligibility to obtain Vermont RN licensure upon acceptance
• Strong case management, documentation, wound care, and IV therapy skills
• McKesson experience preferred
⏱️ Overtime: After 40 hours/week at the all-inclusive rate
