Travel Pharmacist – Outpatient | Arizona
Tuba City, AZ
13-Week Contract | Monday–Friday | Day Shift
We’re hiring an experienced Pharmacist for an outpatient assignment in Arizona, providing comprehensive medication management and direct patient-care services across a diverse patient population.
🔹 Requirements:
• Minimum 3 years of Registered Pharmacist experience required
• Minimum 1 year of Clinical Pharmacist experience required
• Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree from an accredited/recognized program
• Current, valid, and unrestricted U.S. Pharmacist License
• Current AHA BLS certification
• Strong clinical pharmacy and medication-management skills
• Experience providing direct patient-care services
• Ability to work with neonatal, pediatric, adolescent, adult, geriatric, oncology, and obstetric patients
• Strong clinical judgment, communication, and documentation skills
• Must successfully complete required fingerprinting and clearance before start
• Must be comfortable working in a multidisciplinary healthcare environment
🔹 Assignment Details:
• 13-week contract
• Monday–Friday
• Day shift options: 8:00 AM–5:00 PM or 9:00 AM–5:00 PM
• 2 positions available
• Start Date: September 21, 2026
• Allscripts EMR
• No on-call
• Holiday coverage required
• Orientation held every 2 weeks
• W-2 employment only
• COVID & flu exemptions may be considered with required declination documentation
🔹 Key Responsibilities:
• Provide comprehensive pharmacy and medication-management services
• Review, evaluate, and manage medication therapy for patients
• Provide direct clinical pharmacy services in outpatient and clinical settings
• Collaborate with physicians, nurses, and other healthcare professionals
• Counsel patients regarding medications, dosing, administration, and potential adverse effects
• Monitor medication effectiveness and patient outcomes
• Support safe and appropriate medication use across various patient populations
• Maintain accurate pharmacy and patient documentation
• Follow pharmacy policies, procedures, and regulatory requirements
• Participate in clinical pharmacy programs and quality-improvement initiatives
• Assist with development and implementation of pharmacy services
• Support medication-related education for patients and healthcare staff
• Maintain professional standards and patient confidentiality
• Contribute to efficient, high-quality pharmacy operations
