Requirements
Minimum 3 years of Acute Care Case Management experience – REQUIRED
Active Ohio RN License – REQUIRED
Current BLS/BCLS – REQUIRED
Strong experience with acute medical and surgical patients
Discharge planning and care coordination experience
Experience assessing discharge needs and barriers to safe discharge
Knowledge of Social Determinants of Health (SDOH)
Experience with referrals to appropriate post-acute services
Strong communication and interdisciplinary collaboration skills
Traveler must reside outside a 60-mile radius of the assignment

Key Responsibilities
Assess patient discharge needs and develop appropriate discharge plans
Coordinate care for acute medical and surgical patients
Identify and address social determinants of health affecting discharge
Coordinate referrals to post-acute care and community resources
Collaborate with physicians, nurses, social workers, and other interdisciplinary teams
Monitor patient progress and facilitate timely, safe transitions of care
Communicate discharge plans effectively with patients and families
Ensure continuity of care throughout the transition from hospital to appropriate next level of care
Maintain accurate case management documentation
Manage an average caseload of approximately 20 patients per day
