# 🏥 Travel Hospice RN – Admission Visits | California
**13-Week Contract | Days | 40 Guaranteed Hours/Week**

📍 **Territory:** San Francisco County, San Mateo County & Santa Clara County
⏳ **Contract:** 13 Weeks
🕐 **Shift Options:**
* **Sunday–Thursday:** 10 AM–7 PM or 11 AM–8 PM
* **Tuesday–Saturday:** 10 AM–7 PM or 11 AM–8 PM
* Schedule may be flexible based on patient availability
* **1 weekend per month**
⏱️ **Guaranteed Hours:** 40 Hours/Week
💻 **EMR:** Homecare Homebase (HCHB)
🎉 **Holiday Coverage:** Required

### Requirements
* Active California RN License
* Minimum **3 year of Hospice experience**
* Current **BLS/BCLS**
* Strong assessment and care-planning skills
* Ability to coordinate care with a multidisciplinary team
* W-2 employment only

### Key Responsibilities
* Admit patients to hospice services
* Assess physical, psychosocial, environmental, safety, and developmental needs
* Develop individualized plans of care
* Document patient assessments, observations, and interventions
* Coordinate care with the multidisciplinary team
* Conduct follow-up visits as needed
* Participate in quality assessment and performance improvement activities

### Assignment Details
* 13-week travel contract
* Day shift
* Two schedule options available
* One weekend per month
* Holiday coverage required
* No on-call requirement
* Overtime after 40 hours/week
* Mileage reimbursement provided
* 1099 not accepted
