• Shift: Days
• Schedule: 8-hour shifts
• Hours: 40 hours per week
• Contract Length: 13 weeks

Requirements

• Active RN License – Required; specific state license is not mentioned in the posting.
• 1+ year of Hospice/End-of-Life Care Experience – Strongly relevant to the position.
• Experience with patient assessment and care planning.
• Experience with symptom management and medication administration.
• Ability to provide patient and family education.
• Strong communication and care coordination skills.
• Ability to work with physicians and an interdisciplinary hospice team.

Responsibilities

• Provide comprehensive end-of-life care to terminally ill patients.
• Manage an assigned hospice patient caseload.
• Conduct patient assessments and develop individualized care plans.
• Provide skilled nursing interventions.
• Manage symptoms and administer medications.
• Educate patients and families.
• Coordinate care with physicians and hospice support staff.
• Ensure continuity of care and effective communication.
• Evaluate patient condition and effectiveness of the care plan.
