🩺 Specialty: Physical Therapist – Home Health
📅 Contract Length: 13 Weeks
🕒 Schedule: Monday–Friday | 40 Hours/Week + 1 Weekend/Month Rotation

⏰ Guaranteed Hours: 40 Hours/Week

Seeking a Physical Therapist (PT) with home health experience to provide quality patient care throughout the assigned territory.

Requirements
Active California PT License
Minimum 3 year of Physical Therapist Home Health experience
Ability to prioritize Start of Care visits
Ability to supervise Physical Therapy Assistants
Strong communication and timely documentation skills
Responsive during normal business hours
Key Responsibilities
Prioritize and complete Start of Care visits
Supervise Physical Therapy Assistants
Report Start of Care visits by the next day
Complete clinical notes and documentation within 48 hours of visits
Maintain timely communication with supervisors, intake, and office staff
Manage assigned home health caseload and visit requirements
