🏠 Travel Physical Therapist (PT) – Home Health

📍 Location: Oakland, California
📅 Contract: 13 Weeks
☀️ Shift: Days
⏱️ Guaranteed Hours: 36 Hours/Week

We're seeking an experienced Home Health Physical Therapist (PT) for a 13-week travel assignment serving patients throughout the East Bay. This role involves evaluating patients, developing individualized treatment plans, and providing therapy services in a home health setting.

✅ Requirements
Active California PT License
Minimum 3 years of Physical Therapy experience
Home Health Physical Therapy experience
Current BLS Certification
Valid Driver's License
Current auto insurance and vehicle registration
Annual TB screening
Current MMR, Varicella, Hepatitis B, Tdap, and Flu vaccinations
Ability to travel between patient visits and service areas
Strong clinical assessment, communication, and patient-education skills
🏠 Clinical Experience
Evaluate patients and develop individualized treatment plans
Provide therapy focused on improving strength, range of motion, balance, safety, and functional mobility
Educate patients and caregivers on home exercise programs
Monitor patient progress and adjust treatment plans as needed
Provide documentation and care coordination related to home health visits
Communicate effectively with patients, caregivers, physicians, and other healthcare professionals
Provide care across assigned East Bay service areas, with occasional travel outside the primary territory as needed

No on-call requirement
Mileage reimbursement available for travel between patient visits
Weekend coverage may be required when needed to maintain weekly visit expectations
75-mile radius requirement applies
