We are seeking an experienced **Clinical Pharmacist** for a hospital-based assignment. The ideal candidate will have inpatient pharmacy experience and be comfortable working varied shifts, including evenings and weekends.

### Requirements:

✅ Active Ohio Pharmacist License in good standing
✅ Minimum 3 year of inpatient hospital pharmacy experience
✅ Experience with pharmacokinetic dosing, including **Vancomycin** management
✅ Experience with **Warfarin** dosing and monitoring
✅ EPIC experience preferred (Epic Willow Inpatient a plus)
✅ Ability to work evenings, weekends, and varied schedules

### Assignment Details:

📆 13 Weeks
🏥 Acute Care Hospital Setting
⏰ Typical Shift: 12:30 PM – 9:00 PM (may vary)
📋 Weekend coverage required as needed
🖥️ EMR: EPIC

### Responsibilities:

• Review and verify medication orders
• Monitor and manage pharmacokinetic dosing protocols
• Manage anticoagulation therapy, including warfarin dosing
• Collaborate with physicians, nurses, and interdisciplinary teams
• Ensure safe and effective medication therapy for hospitalized patients

CloudinfraIT Inc Job ID #300203. Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
