🦾 Travel Occupational Therapist (OT) – Skilled Nursing Facility

📍 Location: Monterey, California
📅 Contract: 13 Weeks
☀️ Shift: Days
⏱️ Hours: 38 Hours/Week
💻 EMR: PointClickCare / NetHealth (Rehab Optima)

We're seeking an experienced Occupational Therapist (OT) for a 13-week travel assignment in a Skilled Nursing Facility (SNF) setting. The OT will provide rehabilitative services focused on improving patients' functional abilities, independence, and quality of life.

✅ Requirements
Active California Occupational Therapist License
Minimum 3 years of Occupational Therapy experience
At least 1 year of recent SNF experience
Experience with NetHealth (Rehab Optima)
Graduate of an accredited Occupational Therapy program
Strong patient assessment and treatment-planning skills
Ability to develop individualized rehabilitation plans
Excellent communication and interdisciplinary collaboration skills

🩺 Clinical Experience
Perform comprehensive occupational therapy evaluations and reassessments
Develop and implement individualized plans of care
Provide therapeutic interventions to improve activities of daily living and functional independence
Assess patients' mobility, coordination, strength, cognition, and ability to perform daily tasks
Provide patient and caregiver education
Monitor treatment effectiveness and modify interventions as appropriate
Document evaluations, treatments, progress, and discharge recommendations accurately
Collaborate with nursing staff, physicians, and other rehabilitation professionals
Promote patient safety and appropriate use of adaptive equipment
Maintain accurate documentation using NetHealth (Rehab Optima)
Follow infection control, regulatory, and facility standards
