Occupational Therapist – Home Health | Fort Dodge, IA

Location: Fort Dodge, IA
Facility: UnityPoint at Home
Contract: 13 Weeks

Job Summary

Provide skilled occupational therapy services to patients in their homes following hospitalization or skilled nursing stays. Evaluate patients, develop individualized treatment plans, and collaborate with the interdisciplinary home health team to improve independence, safety, and functional outcomes.

Responsibilities
Perform comprehensive occupational therapy evaluations in the home setting
Develop and implement individualized treatment plans
Provide therapeutic interventions to improve activities of daily living and functional independence
Educate patients and caregivers on adaptive equipment, home safety, and home exercise programs
Document patient care accurately in Epic
Collaborate with physicians and the interdisciplinary home health team
Travel throughout the assigned coverage area to provide patient care
Required Qualifications
Minimum 2 years of Occupational Therapy experience
Active Iowa Occupational Therapy License (Compact accepted)
Current BLS Certification
Valid Driver's License
Personal vehicle for travel (mileage reimbursed)
One supervisory reference at submission
Current skills checklist (within one year)
Preferred Qualifications
Home Health experience (training provided if needed)
Epic EMR experience
First-time travelers welcome
Facility Highlights
Average 4–6 patient visits per day
Coverage area within approximately 50 miles of Fort Dodge
Average travel of 200–250 miles per week (mileage reimbursed)
Structured onboarding including Epic training, ride-alongs, and dedicated therapy manager support
Weekly virtual team meetings and monthly in-person meetings
Expected to be fully independent by approximately week 3
