Qualifications
Active OT license in the state where services will be provided.
Graduate of an accredited Occupational Therapy program.
Minimum 1 year of Home Health experience within the past 3 years (required).
Current CPR/BLS certification.
HCHB (Homecare Homebase) EMR experience required.
Reliable personal transportation required.
Strong clinical, communication, and organizational skills.
Key Responsibilities
Evaluate patients in their homes and develop individualized treatment plans.
Provide one-on-one occupational therapy to improve functional independence and daily living skills.
Educate patients and caregivers on home exercises, adaptive equipment, fall prevention, and home safety.
Complete timely and accurate documentation using HCHB EMR.
Coordinate care with physicians and the interdisciplinary team.
Manage up to 6 patient visits per day and meet the 30 productivity-point standard.
Comply with applicable licensure requirements and Home Health regulations.

PDTXperts Job ID #PDSU-0017. Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
