Submission Requirements

• Candidates with an Arizona driver's license will not be considered.
• Provider must have a minimum of 24 months of current wound care experience.
• Certifications must be from the American Heart Association or American Red Cross.
• License verification must show no issues at the time of submission.
• Requested Time Off (RTO) cannot be approved within the first week of the assignment or during holidays and must not exceed 7 days.
• Flu vaccination is required, with the possibility of declinations (forms available upon request).
• Please add vaccination status in the submission notes.

Profile Requirements

• A cover page must be completely filled out.
• Resume should include all facilities, facility size, number of beds, trauma level, and units listed.
• A skills checklist specific to wound care is required.
• Active Arizona or Compact state license is required.
• Active American Heart Association ACLS and BLS certifications are required.
• A valid driver's license must be provided.

Additional Information

• Compliance with specific background requirements is mandatory.
• Travel experience is preferred.
• Weekend and on-call schedules will be determined by the facility.
