Description

Requirements:

  • Certified Home Health Care Experience: Required
  • Experience with EPIC or Netsmart: Preferred
  • OASIS experience: Required
  • BLS Certification: Required

Job Responsibilities:

  • Direct Patient Care Duties
  • Develop and revise plans of treatment and discharge plans in collaboration with patients, their families, physicians, care delivery teams, and third-party payors.
  • Complete OASIS assessments, reevaluations, transfers, resumptions of care, discharges, and assessments of significant changes in condition within defined timeframes.
  • Process orders and notify physicians of patient needs and condition changes. Complete certification/recertification orders and discharge summaries.
  • Promote respect for patients, ensuring their privacy, confidentiality, and property are safeguarded.
  • Initiate appropriate preventive and rehabilitative strategies and provide specialized nursing services.
  • Consult and educate patients, families, and team members about the disease process, self-care techniques, and preventive strategies.
  • Supervise LPNs to ensure patient health, safety, and care plan compliance.
  • Direct, coordinate, evaluate, and supervise the quality of patient care services provided.
  • Ensure completion of clinical records required for state and federal compliance and seamless service delivery.
  • Maintain knowledge of available community-based services.
  • Resolve problems at the point of contact and secure authorization and release of information from patients or responsible parties.
  • Attend patient care conferences.

  • Agency Operational and Financial Objectives

  • Monitor results, progress, and take corrective action on quality indicators, focusing on customer satisfaction.
  • Understand the impact of nursing on patient outcomes, readmissions, productivity, and financial metrics.
  • Pursue cost reduction and error elimination efforts.
  • Respond to quality management reviews, ensuring problem resolution.
  • Participate in peer review and quality assessments when assigned.
  • Maintain productivity standards.
  • Ensure appropriate visit authorizations are in place, and visit documentation demonstrates medical necessity for home care.

  • Responsible for meeting Key Performance Indicators (KPIs).
  • Behavioral Characteristics and Skills:

    • Display strong interviewing, clinical assessment, and communication skills.
    • Demonstrate initiative and independent judgment.
    • Build and maintain professional relationships.
    • Hold a current NYS driver's license.
    • Work independently with minimal supervision, adapt to change, and manage multiple assignments.
    • Certified Home Health Care experience, OASIS, and telemetry experience are required.
