Description

Requirements:

Certified Home Health Care experience required
EPIC or Netsmart experience preferred
OASIS experience required

Certifications:

BLS required

Job Responsibilities:

Care Coordination:

Develop and revise treatment and discharge plans with care teams.

Complete OASIS assessments and manage patient care documentation.

Process orders and update physicians on changes in patient condition.

Create a respectful environment for patient privacy and property.

Initiate preventive and rehabilitative strategies and provide specialized nursing care.

Supervise LPNs to ensure patient safety and compliance with care plans.

Direct and evaluate patient care quality provided by the Home Health Agency.

Maintain positive interactions with healthcare professionals and patients.

Engage in care management philosophy promoting quality outcomes.

Operational and Financial Contribution:

Monitor quality indicators and customer satisfaction.

Contribute to cost reduction efforts and address quality issues.

Participate in peer review and quality improvement assignments.

Maintain productivity and ensure authorization for home visits.

Professional Development and Environment:

Promote workforce diversity and participate in educational programs.

Demonstrate commitment to professional growth and competency.

Fulfill additional duties as requested and uphold agency objectives.

Skills Required:

Clinical nursing experience in hospital and home care settings.
Critical thinking and clinical judgment.
Strong interviewing, assessment, and communication skills.
Initiative and independent judgment.
Ability to build professional relationships.
Current New York State driver's license.
Ability to work independently, adapt to change, and manage assignments.
Telemetry experience required.
