Responsibilities
- Perform therapeutic procedures under supervision, adhering to established protocols.
- Follow therapeutic plans, incorporating functional goals and therapeutic modalities.
- Seek input from residents and caregivers, incorporating feedback into treatment sessions.
- Report problems, questions, or changes in resident status to the evaluating therapist promptly.
- Attend relevant team meetings.
- Monitor and adjust treatment programs to meet functional goals.
- Document all therapeutic interactions in a timely manner, ensuring accessibility to the care team.
- Recommend and instruct on the use of assistive devices for residents.
- Provide education to residents or caregivers and document teaching outcomes.
- Comply with Medicare, Medicaid, and reimbursement regulations.
- Support the development of new rehabilitation programs and services.
- Identify learning needs and pursue educational opportunities.
- Maintain cleanliness and orderliness of department areas and equipment. Follow infection control procedures.
- Keep required licenses, certifications, and competencies up to date and complete annual compliance courses.
Job Requirements
Knowledge and Skills:

Education and Experience:
- Associate degree required.
- PT Assistant License required.
- Basic Life Support (BLS) certification required.
- Prior experience in a long-term care or skilled nursing facility preferred.
Working Conditions/Physical Demands
- Ability to sit, stand, walk, bend, stoop, twist, and reach as required.
- Must be able to lift up to 50 pounds; lifting above 50 pounds requires assistance.
- Frequent lifting/carrying up to 20 pounds.
- Manage stress related to ensuring quality care.
