RN Case Manager – Milwaukee County
Up to $7,500 Sign-On Bonus | Full-Time, Day Shift
Join our team and make a meaningful impact every day. We’re looking for a compassionate Registered Nurse in the Homeheath field in Milwaukee County. In this role, you'll build strong patient relationships, collaborate with a supportive team, and help families navigate an important life journey.
RN Home health Key Responsibilities:
Perform initial home health nursing evaluations within 24 hours of hospice referral.
Assess physical, emotional, and environmental needs of patients and families every 15 days or as needed.
Develop, implement, and revise individualized care plans based on patient needs.
Collaborate with the interdisciplinary team, attending physicians, and families to deliver comprehensive care.
Document patient progress, goals, and interventions accurately and timely.
What You’ll Need:
RN license (Wisconsin) + 1 years of home health or hospice experience
Valid driver’s license and reliable transportation
