Description
Location: Racine/Kenosha
Up to $75000/yr with Master’s in Social Work
DUTIES & RESPONSIBILITIES
Providing emotional support and counseling to hospice patients and their families
Assessing patients’ psychosocial, emotional, financial, and environmental needs
Helping patients and families cope with terminal illness, grief, and end-of-life planning
Coordinating community resources and support services, such as financial assistance, counseling, or placement services
Assisting with advance directives, living wills, and healthcare decision-making
Collaborating with nurses, physicians, chaplains, and the interdisciplinary hospice team to develop care plans
Educating families on hospice services, patient rights, and available resources
Conducting crisis intervention and conflict resolution when needed
Providing bereavement support and follow-up services to families after a patient’s passing
Maintaining accurate documentation and ensuring compliance with hospice regulations and standards
Advocating for patient comfort, dignity, and quality of life throughout hospice care
JOB REQUIREMENTS
Must have a bachelor’s, master’s, or doctoral degree from a school of social work accredited by the council on Social Work Education.
Must be a licensed social worker in the State of WI or have the ability to be licensed within one year of hire.
Minimum of one year's social work experience in health care setting preferred.
Experience in home health care and/or hospice care preferred.
Demonstrates good verbal and written communication and organization skills.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
